Healthy Living Program
The Bogan Shire Medical Centre focuses on Chronic Disease Management (CDM) as part of our Healthy Living Program. Our program enables our team to plan, monitor and coordinate the health care of our patients with chronic medical conditions. This care is provided by a care team including the Doctor, a care coordinator, a practice nurse and other health care providers, such as our Podiatrist.
If you suffer from any chronic, long term health issues, the Bogan Shire Medical Centre can support you through our Healthy Living Program.
It is the Bogan Shire Medical Centre Policy to ensure that patient health information is managed in a secure manner. Patient records are electronic and kept on a separate and secure server in keeping with the Privacy Act 1988 and Australian Privacy Principles. Please see our brochure on “Privacy & Your Rights” in you would like further information.
It is a requirement by lay that you identify yourself to reception staff and other members on the practice team at each contact using the practice 3 identifiers. You will be asked Name, Date of Birth and Address.
The Bogan Shire Medical Centre does not offer communication via Email, SMS or Website in relation to patient care. It is our policy that a message will be taken by the care coordinators and delivered to the Doctor at a time deemed appropriate during the working day. Wither the Doctor or practice staff will return your call with a response. If the call is urgent and prior notice given to the practice staff from the Doctor the call will be directly put through.
As a patient of the Bogan Shire Medical Centre your results are imported into our practice computer software system. These results are only accessed by our Doctor. The Doctor reviews results regularly throughout the day. You may be asked to return for your results at the time of your consultation or notified by the Doctor or practice staff to discuss your results.
